• assistance with splints and orthotic devices
• hand function and fine motor rehabilitation
• bathroom safety evaluation

Medical Social Services

• assistance in accessing community resources for finances, food and transportation
• counseling
• assistance in obtaining respite care and other health care alternatives
• long-range planning for alternative placement assistance

Home Health Aide Services

• bathing and grooming
• dressing
• transfers and walking
• assistance with other daily activities

Eligibility
Home health services are reimbursed by Medicare, Medicaid, or private insurance depending on the level of care and the individual’s insurance plan.

MEDICARE:  Home Health services are 100 percent reimbursable when the patient meets homebound criteria and when a physician orders services requiring intermittent skilled nursing, physical therapy or speech therapy.

MEDICAID:  Payment is determined by person's program eligibility.

PRIVATE INSURANCE:  Wesley Homes At Home can assist in determining insurance coverage.
